What is Lifeways?

The Lifeways program provides expert guidance and practical support for anyone in Anchorage with a life-threatening diagnosis, as well as their caregivers and other loved ones. As a Lifeways client, you keep your own doctor, and continuing treatment does not affect your eligibility. We meet you where you are and work with you to develop a care plan that meets your priorities and needs to maximize your quality of life. All services are provided without charge to you. Lifeways services are designed to help you regain some control when life can seem out of control and allow you and your family to focus on the most important things.
 
Lifeway Services May Include:
Hospice of Anchorage

NURSE EDUCATOR:  Nurse consultation and education about symptom management and caregiving.

COORDINATION:  Coordination with your physician and other professionals to ensure smooth communication and effective care.

SUPPORT:  Emotional, spiritual, and grief support, as desired, for both you and those close to you. Grief support can be ongoing for your loved ones.

ASSISTANCE: Assistance navigating financial, legal, and planning issues from a qualified social worker.

CARE ASSISTANT:  Caregiver assistance provided by professional personal care assistants (PCAs) supported by the Alaska Senior In-Home Services program for qualifying families.

COMPASSIONATE COMPANION:  Compassionate companions are trained volunteers who do a variety of activities dependent on client interest. Some volunteers provide specialized services such as massage or pet therapy.

Compassionate Care

The Hospice of Anchorage team of volunteers, nurses, social service coordinators, and bereavement specialists work with our patients and families, along with the patient’s physician supporting the transition from life through death and coping with loss and grief. This may be a one-time visit, periodic contact, or may grow into an ongoing relationship lasting weeks or months. All services are provided free of cost.
 
Hospice care offers people with life-limiting illnesses the opportunity to live fully and support families and other loved ones in coping with illness’s effects and challenges.

Assistance with insurance, financial and legal matters, and advance directives (living will, durable power of attorney, and Comfort One)
